To set it up I highly recommend using Nova Launcher. Change the grid size to 10 high by 8 wide. Then you add the widgets and pick the skins. For the Weapon Wheel to pop out like in the video you need PopUp Widget.
I will add a how to tutorial in the near future. For now if you have any questions you can email me from within the app and I'll walk you through the process.
